Home visits

Please remember that home visiting is very time consuming for the doctors and we would be grateful if patients would come to surgery unless they are too ill or disabled to do so. If a visit is necessary, it should be requested before 10.45am where possible.

Repeat prescriptions

Patients on regular medication do not always need to see a doctor for a repeat of their medication. We require 48 hours notice (not including Saturday and Sunday) for a repeat pre­scription. Repeat prescriptions can be collected from Recep­tion during surgery hours, or we can post them to you if you include a stamped addressed envelope. You can continue to request repeat prescriptions up to the review date that appears at the bottom of the prescription request slip. You should book to see the Doctor for a review before the review date ar­rives, as repeat prescriptions will not normally be issued be­yond an expired date.

Change of name, address & telephone numbers

Please inform us of any change of name, address or tele­phone number to enable us to maintain accurate records about you.

Citizens Advice Bureau

The Citizens Advice Bureau staff member visits the Practice on alternate weeks. If you have problems with Benefits, Housing or Community Care, please book an appointment for advice.

Emergencies

On evenings, at night and on weekends when emergencies arise outside of surgery hours, telephone the Practice on 0845 077 0023 in the usual way. Your call will be diverted to PRIME CARE the out of hours duty service where your emergency will be addressed.

Should you require non-emergency telephone advice from the doctor or the practice nurse, it is best to try between 10.45-11.45am where possible or between 5.30-6.00pm. Please appreciate that it may be necessary to ask you to either hold or ring back, should the doctor be busy with another patient.

Confidentiality

All our staff are governed by a confidentiality policy and no information contained in your medical records will be revealed to anyone outside of the primary health care team without your permission.
